NSIPA is a nephrology medical group and care management support professionals that provides chronic and end-stage kidney disease patients with a more complete kind of care. Our approach is value-based, which means that we collectively work toward better health care outcomes for you. 

Approximately 37 million patients suffer from chronic kidney disease (CKD), and more than 726,000 patients have end-stage renal disease (ESRD). Early detection and optimal starts for your treatment can have a significant impact on your longterm health. We work with you to navigate the life changes that can come with both CKD and ESRD. 

Our Care Management Program provides 24/7 support for our patients with board-certified emergency medicine physicians, staff nurses, social workers, dietitians and more. This is done through virtual-first care with robust care coordination, and by removing obstacles to care caused by social determinants of health. 

Kidney disease can significantly change your life day-to-day. NSIPA proactively helps you coordinate the many aspects of your care management. We offer transitional care visits that work to ensure that you are safe in your outpatient environment, reconcile your medications, and assisted you with any necessary care coordination.

There are a few dialysis options available; peritoneal, hemodialysis, in-home, or on-site. The method used is dependent upon the patient and their unique needs. We work to find the best solution for each patient. For example, we provide dialysis on wheels, which is an ambulatory dialysis service where a vehicle is sent to the patient’s home because that person cannot effectively travel to the dialysis center.
